How successful is your hospital care management program?  An effective care management program is essential to provide effective care transitions for your patients and securing appropriate reimbursement.

While health care challenges continue to evolve and disrupt our best-laid plans, it’s important that we allow the challenges to guide and strengthen our processes. Effective and consistent care management processes, such as care coordination and discharge planning, support our patients along the continuum of care in receiving the right care in the right care setting.  Additionally, you will build your confidence in navigating the day-to-day.

Efficiency in care management processes provides insight into how the whole organization can improve on the most critical issues. Using key metrics to track the progress of processes, understanding  Clinical Documentation Integrity (CDI) and Conditions of Participation (CoP) may increase patient satisfaction. You will learn  financially sound practices of denial management, the two-midnight rule, and patient status management.
