Discharge planning is a process for identifying and preparing for a patient’s anticipated healthcare needs and touches every patient and their care givers. Planning and coordinating care are essential to the patient’s transition from one level of care to the next. This webinar will discuss how care coordination amongst the care team and inclusion of a patient in their discharge planning can adequately and efficiently improve a patient’s quality of life by ensuring continuity of care and reducing the rate of unplanned readmissions and/or complications.
